Print It Out and Complete It
After downloading our form, print it out and complete all of the sections. Make sure to note that the form is two pages in length.

Bring It In
Bring your completed form (and your pet!) in to see us. If you wish to have the ZnSO4 centrifugation performed, please be sure to bring along a stool sample. Detailed directions to our clinic can be found by clicking here.
